Certificate of registration on the educational register or for fellows
New procedures as of April 1, 2012
As of April 1, 2012, only online applications submitted through a secure transaction platform will be accepted. However, "paper" applications that have already been received or are awaiting processing will be honoured for a period of 90 days, i.e., until July 1, 2012. After this date, paper applications will no longer be processed.
Therefore, as of July 1, 2012, all applications for a certificate of registration on the educational register be made online, as well as applications for a certificate of professional conduct submitted in order to obtain privileges in an institution. Furthermore, the fee must be paid by the applicant at the time of application.
This change means that as of July 1, 2012, the CMQ will no longer process applications made using the AS-227 form. All applications must be made using the transaction platform.
Fees
- Medical students with a valid registration certificate: no fee
- Residents and fellows with a valid training card: $60
- Candidates who were residents or fellows in Québec, but no longer have a valid training card: $150
- Physicians registered on the CMQ roll: $60
- Physicians not registered on the roll (ex-CMQ members): $150
Terms and conditions of payment
Fees must be paid at the time of application and are payable only by credit card (Visa® or MasterCard®).
This change means that deferred payment will no longer be accepted for applications for certificates submitted by an institution. Physicians who apply for a certificate of professional conduct in order to obtain privileges in an institution must pay the required fees themselves at the time of application.
Processing of applications
Applications are processed on a first-come, first-served basis. Normal processing time is around 10 business days.
As soon as the certificate has been issued, the student, resident, fellow or physician who submitted the application will receive an email informing him/her that the certificate has been issued and where to find a copy of the certificate on his/her personal secure page.
