Required documents

You must furnish an official translation of documents written in a language other than English or French, that is, a translation bearing the seal of an official translator. Separate translations must be attached to each corresponding document. All documents must be letter-size (216 mm x 279 mm or 8.5 in x 11 in).

Please take note that original documents will not be returned. It is recommended that certified true copies of original documents be sent to the Collège.

• Birth certificate
• Marriage certificate (in case of change of name)
• Medical degree
• Documents attesting to postgraduate training (residency). Each document must be signed by the Dean of the faculty of medicine or the Executive Director of the establishment where the training was done and must specify the contents as well as the start and completion dates of the training in each discipline.
• Certificates and Diplomas Obtained
• Certificate of good standing or membership in good standing issued by each of the medical orders under which the candidate practiced (if applicable).
