Diploma and postgraduate training in the United States

The list of documents required by the Collège des médecins du Québec (CMQ) is made of three parts.

Part 1

Documents to send to the PCRC which will be source-verified by the organization in accordance with established criteria
  • Photograph;
  • Proof of identity;
  • Certificate proving a name change, if applicable;  
  • Medical Diploma (M.D.);
  • Attestations of postgraduate training periods (residency training) signed by the dean of the faculty of medicine or the executive director of the establishment where the training periods were served. These attestations must specify the dates on which they began and ended for each discipline;
  • Specialist certificate(s) (including family medicine);
  • Permit to practice medicine obtained outside of Québec, if applicable.

By mail

The following documents must be sent by mail to the Collège des médecins du Québec – permits section as soon as your application for a permit on line is completed.

IMPORTANT — Please take note that original documents will not be returned. Certified true copies of original documents must be sent to the Collège. You must also furnish an official translation of documents written in a language other than French or English, that is, a translation bearing the seal of an official translator. Separate translations must be attached to each corresponding document. All documents must be letter-size (216 mm x 279 mm or 8.5 in x 11 in). 

  • Certificate of professional conduct issued within the last three months by the medical order of each country in which you practiced medicine, by a State Board or a college of physicians in a Canadian province where you hold or held a permit to practice medicine. Each certificate must be sent directly to the CMQ by the organization under sealed fold. These certificates must be written in French or English;
  • Attestation(s) of recent medical activities signed by the program Director or by the chief of department;
  • Official document proving knowledge of the French language, if needed.

In accordance with the terms of The Charter of the French Language, in order to be exempted from the examination of the Office québécois de la langue française (OQLF), you must submit one of the following proofs:

  • a Medical Degree (M.D.) from a university whose language of instruction is French; or
  • an official document (record of marks) proving that you have followed a full time course of instruction starting from the secondary level for at least three years in a French language institution; or
  • an official document (record of marks) from the ministère de l’Éducation, du Loisir et du Sport proving that you have obtained a certificat d’études secondaires from Québec after the 1985-1986 school year; or
  • an official document (record of marks) from the ministère de l’Éducation, du Loisir et du Sport proving that you have succeeded the "Français, langue maternelle" examination in your fourth or fifth grade in the secondary level course*.
