Required documents

The list of documents required by the Collège des médecins du Québec (CMQ) is made of two parts.

Part 1

Documents to send to the PCRC which will be source-verified by the organization in accordance with established criteria
  • Photograph;
  • Proof of identity;
  • Certificate proving a name change, if applicable;  
  • Medical Diploma (M.D.);
  • Attestations of postgraduate training periods (residency training) signed by the dean of the faculty of medicine or the executive director of the establishment where the training periods were served. These attestations must specify the dates on which they began and ended for each discipline;
  • Specialist certificate(s) (including family medicine);
  • Permit to practice medicine obtained outside of Québec, if applicable.

Part 2

Documents to send to the Collège des médecins du Québec. Certified true copies of original documents must be sent, if applicable

  • Recent photograph (less than 12 months old), passport type (50 mm x 70 mm) with your signature, and authenticated by a witness who signs at the back. Any person who can confirm your identity is authorized to sign;
  • Application for eligibility for a restrictive permit form, completed and signed;
  • If you do not have a financial agreement with Recrutement Santé Québec, a cheque made out to the Collège des médecins du Québec for the required amount (non-refundable) covering the costs of processing the file;
  • Proof of successful completion of the Medical Council of Canada Evaluating Examination (MMCEE) or Standard Certificate of the ECFMG or the USMLE / Step 2 CK. Any application for eligibility sent to the CMQ without a proof of success to one of these exams will be deemed unacceptable and will be returned to the candidate;
  • Curriculum vitae;
  • Certificate of professional conduct issued within the last three months by the medical order of each country, State Board or Canadian province in which you are practising or have been practising medicine. Each certificate must be sent directly to the CMQ by the organization in sealed envelope. These certificates must be written in French or English;
  • Attestations of medical activities over the last two years;
  • Two letters of recommendation written in the last 12 months, from professors, program directors, department heads or colleagues, from outside of Québec.
    These letters should be:
    - dated;
    - printed on letterhead of the Hospital (not handwritten);
    - signed and contain, under the signature, the signer's name and title.
    If the seal applied is in a language other than English or French, it must be translated by an official translator.

For all documents submitted, you must furnish an official translation of documents written in a language other than English or French, that is, a translation bearing the seal of an official translator. If the wording of the seal is in a language other than English or French, it must also be translated. Separate translations must be attached to each corresponding document. All documents must be letter size (216 mm x 279 mm or 8.5” x 11”), except for the photograph.
