In this section, you will find a list of all the documents required for the different types of applications submitted to the Collège des médecins du Québec (CMQ).

These documents will be source verified in accordance with established criteria. Please make sure that you have authorized the CMQ to access all your documents. The CMQ will not accept provisional degree certificates (M.D. and specialized programs).

Translation of documents submitted to physiciansapply.ca
If your credentials are in a language other than English or French, you are required, since October 3, 2016, to use the translation service offered through your physiciansapply.ca account. The option to supply your own translation is now discontinued.

Documents that must be sent directly to the CMQ
Copies must be certified, since documents will not be returned to you. If documents are written in a language other than English or French, you must provide an official translation bearing the seal of a certified translator. If the wording of the seal is in a language other than English or French, it must also be translated. Separate translations must be attached for each document. All documents must be letter size (216 mm x 279 mm or 8.5 in x 11 in).

IMPORTANT - The Medical Education Division will not keep any document received on your behalf before the reception of your request from physiciansapply.ca or the CMQ Online Services.

Letter from the Director of Professional Services
Letter required:

  1. For practice in an institution: letter from the Director of Professional Services specifying the dates, disciplines, activities that will be practiced and the name of the responsible department head.
  2. For practice at an accredited training site: letter from the Director of Professional Services specifying the dates, disciplines, activities that will be practiced and the name of the responsible department head and the name of the Dean of the Faculty of Medicine.
  3. (Exclusive to telemedicine) For practice in another organization: letter of justification from the medical director or medical sponsor specifying the dates, disciplines, activities that will be practiced, the name of the responsible medical director or medical sponsor; this is in addition to the letters required under Points 1 and 2, if applicable.

* A déclaration sur l'honneur certifying that to your knowledge, you are not the subject of any proceedings that could lead to a conviction or sanction that might have an impact on your registration with the CMQ.

N.B. A letter of support from Recrutement Santé Québec (RSQ) for priority specialities, if applicable, must be sent directly to the CMQ by the organization concerned.

* If you are in your final year of training or have completed your postgraduate training and are waiting to receive your State Doctor of Medicine degree and/or your specialist’s certificate, you must send a letter to the Dean of the Faculty of Medicine indicating the expected date of issue of the degree and/or certificate. 

(1) The Policy respecting a professional’s name and name change requests of the Collège des médecins du Québec (CMQ) requires,  at the time of first registration with the Collège, that you as the professional provide two identity documents (IDs) and validate your usual professional name. For the  first ID, the professional born in Canada must upload a certified true copy of his birth certificate (issued by the Registrar of Civil Status) and the professional born outside Canada, a certified true copy of his valid passport. To complete the validation of your first ID, please provide by regular mail to CMQ a certified true copy of this ID. The second ID must be identical to your usual professional name (see the Policy).

(2) Certificate(s) of professional conduct no more than three months old, issued by the medical regulatory authority of every country, State (State Board) or province that you are or have been licensed to practice in. Each certificate must be sent directly to the CMQ by the issuing authority, under seal.

(3) Postgraduate training certificates (corresponds in some countries to internship or residency): The letters must be signed by the Dean of the Faculty of Medicine or by the Chief Executive Officer of the institution where the training was completed. The start and end dates must be specified for each discipline. 

(4) Certificates of medical activities:* Provide details about the type of activity, the duration (number of hours/week and number of months/year).

(5) Letters of recommendation:* Letters from professors, program directors, department heads or colleagues, from outside Quebec.

*These documents must be dated and printed on hospital centre letterhead (typewritten). They must be signed and the name and title of the signatory must appear under the signature. If the seal is in a language other than English or French, it must be translated by a certified translator. 

(6) Results / exams required: Applicants who are eligible to sit the Medical Council of Canada (MCC) examinations must, without fail, respect the examination registration dates and note the dates the examination results will be available, which are posted on the MCC’s website.
