• See the list of medical services you have received that are paid on a fee-for-service basis

And Quebec Health Record?

The Québec Health Record (QHR) is a communication platform that facilitates the secure and timely sharing of health information between authorized organizations and stakeholders. It supports their delivery of health services for the benefit of users.

Authorized providers may access and consult the information in your QHR. Therefore, the QHR cannot be accessed by all health and social services providers.

When you consult a health and social services professional (family medicine group, CLSC, emergency room, etc.), your health information is entered in the local record kept at each location where you consult. Some, not all, of the information entered in the local record is automatically shared and stored in the QHR. Only professionals at the location where you consulted can access the information in your local record.

You cannot access the DSQ, but the healthcare professionals and social services professionals you consult can. Therefore, they do not need to obtain a copy of the file created by your previous physician to review your health history.

You do not have to register with the QHR. Unless you refuse, authorized health information is automatically and systematically collected in the QHR. Visit the following web page for more information about the DSQ.

To obtain a copy of the file created by your physician:

a) If you consulted them at a healthcare facility

The archives of healthcare facilities maintain medical records, and the user’s file rules apply.

b) If you consulted the physician at their private office.

The physician who practices off-establishment must maintain your medical records for a period of at least 5 years depending on the date of the last entry or insertion in the record; or the end date of the project, if it is a research project.

After this 5-year period, the record is considered inactive and may be destroyed.

To find out where your physicianr’s medical records are located, as well as the contact information to obtain your file, consult the physicians directory. 

The “File transfer” section in the physician’s profile will indicate to whom and where to send a request for a copy of the file. For example:

c) If your a medical record is held by the CMQ

If your a medical record is held by the CMQ :

• You can obtain a copy free of charge. Print the form below, fill it out, sign it and return it to the CMQ with a photocopy of your health insurance card.

• Please note that your new physician or a third party, such as your insurer or your legal representative, may request your medical records directly. A copy of your file will only be sent to them if they include your written consent with their request.
