Request access to a document

The CMQ is subject to the laws relating to the protection of personal information and access to documents. This section describes the procedure for requesting access to a document held by the CMQ. It may be a document related to monitoring the practice of the profession or a medical record of which the CMQ is the transferee or custodian.
