Application for review of a file for advanced training periods as a fellow
IMPORTANT - Please take note that original documents will not be returned. Certified true copies of original documents must be sent to the Collège.
You must also furnish an official translation of documents written in a language other than English or French, that is, a translation bearing the seal of an official translator. Separate translations must be attached to each corresponding document. All documents must be letter-size (216 mm x 279 mm or 8.5” x 11”).
Applications for a review of file for advanced training periods as a fellow must be sent to the CMQ by the concerned faculty of medicine in Québec, not by the candidate.

- Birth certificate;
- Marriage certificate proving a change of name (if applicable);
- Doctor of medicine diploma (MD), with official translation into English or French (if applicable);
- Permit to practice medicine (if applicable);
- Standard certificate from the Educational Commission for Foreign Medical Graduates and or the USMLE (Step 2) or proof of successful completion of the Evaluation Examination of the Medical Council of Canada (if applicable);
- Specialist’s certificate (if applicable);
- Letters attesting to medical activities (letters of reference);
- Documents attesting the validation of postdoctoral training periods (internship, residency, etc.). Each document must be signed by the dean of the faculty of medicine or by the executive director of the establishment where the periods were served and must specify the starting and finishing dates for each discipline;
- A non-refundable cheque for the prescribed amount payable to the Collège des médecins du Québec, to cover the cost of processing the file (postal order or certified cheque, no ready cash and no personal cheque);
- A copy of the curriculum vitae, including a list of publications;
- A proof of acceptance into a program in family medicine or in a specialty (letter from the dean or from the assistant dean of the faculty of medicine indicating the discipline, the dates and places where the periods will be served);
- A proof of acceptance into a host or exchange program approved by the faculty of medicine or government authority (letter from the government of a country or faculty of medicine or from the Government of Québec or Canada or from an international or scientific organization).
