Access the physicians and residents directory

If you are looking for a physician’s or a resident's contact information and you know their name or permit number, you can consult the directory.

The CMQ makes a directory available to the public with the contact information of all active physicians and residents. Address, specialties, permit type, disciplinary files, member's history; you can find a lot of useful information.

To obtain a physician’s contact information

You must perform a search by specifying one of the following criteria:

  • Their license number (5 digits);
    or
  • Their last name (full or at least 3 characters).

Note

This tool is not the Roll of the Order or a referral service. The CMQ cannot guarantee that a physician will accept new patients.

If you are unable to find a physician’s contact information, you can call the CMQ during opening hours at 514 933 4441 or contact us at info@cmq.org.

Information in the directory

To find a physician’s contact information

You must enter one of the following criteria, then click on “Find a physician”:

  • Their permit number (5 digits);
    or
  • Their surname (full surname or at least 3 characters).

Physicians 

The directory provides the following information for a member in good standing of the CMQ whose name is known:

The directory provides the following information – available at the moment of the consultation – for a member in good standing of the CMQ and former doctor : 

  • Surname, first name
  • Sex
  • Specialty(ies)
  • Permit type
  • Status (and any practice restrictions)
  • Address of the principal place of practice
  • Telephone number
More information available :
  • Information on physicians who are deceased, who have been struck off the Roll, who have resigned or whose permit has been revoked;
  • Nature of the practice restrictions
  • Disciplinary information (in particular active complaints before the Disciplinary Council)
  • Information on the transfer or provisional custody of records
Residents

The directory provides the following information about residents :

  • Surname, first name
  • Training card number
  • Status
  • Faculty
  • Program
History

Decisions modifying the right to practice
Restriction, striking off, suspension or revocation of the permit

  • Active decisions are published in the physician’s directory entry.
  • Past decisions appear in the physician’s history in the right upper corner of their directory entry.

Updating

The information in the directory is updated regularly and is the latest information provided to the CMQ by physicians.

To find a family doctor or a SNPPC

To find a family doctor, a specialized nurse practitioner in primary care (SNPPC) or a group of doctors that is accepting new patients, you can put your name on the waiting list through the Québec Family Doctor Finder (GAMF).
